Plan Information

Employees and their dependents are eligible for health care coverage beginning the first day of employment with the City of Lewiston. The City covers 100% of the premium for the employee and the employee pays 20% of the premium for dependents.

  • Deductible is $1,500 per individual or $3,000 per family
  • Maximum Out-of-Pocket is $3,000 per individual or $6,000 per family
  • Co-pays: 
    • Doctor visit - $30
    • Specialist visit - $50

Effective January 1, 2018, AllianceRx Walgreens Prime became Regence’s new home delivery and specialty pharmacy. AllianceRx Walgreens Prime is a combined specialty and home delivery pharmacy services company formed by Walgreens and Prime Therapeutics.  

Quit for Life Smoking Cessation Program

The City has an additional benefit of a Smoking Cessation Program which includes coverage of prescription medication and/or patches/gum.  Additional information can be found by viewing the Quit for Life (PDF).
