Plan Information
Employees and their dependents are eligible for health care coverage beginning the first day of employment with the City of Lewiston. The City covers 100% of the premium for the employee and the employee pays 20% of the premium for dependents.
  • Maximum Out-of-Pocket is $3,000 for an individual/$6000 per family
  • Co-pay Doctor Visit - $30
  • Co-pay Specialist Visit - $50
  • Out of Network Deductible is $5,000 for an individual/ $10,000 for family
  • Prescription Co-pay  $10 for 90-day supply of Generic Rx
  • There is a separate $200 annual deductible for Formulary Brand, Non Formulary, and Specialty Medications.  
  • Formulary Brand Drug has a minimum $25 copay and maximum $100 copay
  • Non Formulary Brand and Specialty Drugs have a minimum $75 copay and maximum $200 copay
  • Specialty Drugs have a co-pay of 50% up to $300

BriovaRx and OptumRX
Effective March 1, 2016, BriovaRx will be Regence's preferred Specialty Pharmacy, replacing Walgreens. Effective April 1, 2016, OptumRx Home Delivery will be Regence's preferred mail-order pharmacy, replacing Walgreens.

Walgreens is working closely with Regence to minimize disruption to members. Walgreens continues to be an in-network provider for walk-in non-specialty prescription coverage and some Limited Distribution specialty medications.

Quit for Life Smoking Cessation Program
The two most costly health care issues in society today are obesity and smoking.  The City has negotiated the additional benefit of a Smoking Cessation Program which includes coverage of prescription medication and/or patches/gum.  Additional information is available at this link.
